Keel laid February 5,1962, launched July 20,1963, delivered November 4,1964, commissioned November 7,1964. 

Lead ship of Class. Reached 33.6 knots on trials. Equipped with DASH, ASROC, LAMPS and Terrier. Complement of 31 officers and 387 enlisted. Served in the Atlantic, Pacific, Mediterranean and in Vietnamese waters. In the Ionian Sea on November 22,1975 collided side to side with the aircraft carrier JOHN F. KENNEDY, CV-67. The BELKNAP's superstructure burned and melted due to aviation gas leaking from the damaged carrier, 8 dead, 25 injured. The ship was towed to the United States and placed out of commission December 20,1975. Rebuilt at Philadelphia Naval Shipyard beginning January 9,1978, where she was stripped to the bare hull and completely rebuilt to a new design. Recommissioned May 10,1980 and reclassified CG-26. Served as Flag Ship of the 6th Fleet. Served in the Persian Gulf War.
